HSMV Form 71120 (Certification of Address) is not accepted for CDL applications. This form works for standard driver's licenses but is explicitly excluded for commercial license applications — bring one of the four document types above instead.
The Originals-Only Rule
All documents must be original or certified copies — photocopies are not accepted, for either appointment. This is stricter than several other states and catches applicants who assume a printed copy of a lease or utility bill will work. Bring the actual original document or an official certified copy.
